Recovering your Username or Password
If you have never logged in before please follow the steps here.
From your patient portal page, click the blue Get help accessing your account link.
To recover your Username
Select I forgot my username and click the blue Next button.
Enter First Name, Last Name, and Date of Birth and click the blue Next button.
Select whether the username should be emailed or texted and click the blue Next button.
The screen will display a confirmation message that the username has been sent via the selected method.
To recover your Password
Once the username has been recovered, or if the username is already known, navigate back to the home page of your patient portal and click the blue Get help accessing your account link.
Select I forgot my password and click the blue Next button.
Enter the Patient Web Portal username and select the blue Next button.
Answer the account security question.
Once an accurate security answer has been provided, select either Mobile, to have a password reset texted to the mobile number on file, or Email to have the password recovery emailed to the email on file. Select the blue Next button.
The screen will display that a temporary password has been sent via the selected method.
